Court Forms On This Site

Forms help you to prepare the documents that you need in a court process. For example, you need to use forms to start a divorce case, to ask the court to appoint you legal guardian of a child, or to protect you against domestic violence.

You will find almost all of the forms you will need on this website, in the "forms" section about your case type. You can fill them out online, or print them out and fill them out by hand. They are all free of charge.

 

Judicial Council Forms

The Judicial Council of California has created many forms (called "Judicial Council
forms") Opens new window to standardize the preparation of court documents. On this site, you can use these forms for your case free of charge.

  • The forms and instructions for the Judicial Council forms are in Adobe Acrobat (PDF) format. If you already have Adobe Acrobat Reader installed on your computer, you can download forms and instructions and print them out.
  • If you don't have Adobe Acrobat Reader on your computer, visit the Adobe site Opens new window to download a free copy.
  • Once Adobe Acrobat is installed, you can download a form by clicking on the form number. The form will appear on your screen. Then, you can print it and fill it out.
  • Then, click the "Back" button to return to the page you were on. If there are instructions for the form you need, you can download them, too.

People involved in lawsuits (also called "litigants") must use Judicial Council forms that are labeled "mandatory use" and may use forms that are labeled "optional use." To see if your form is mandatory or optional, look for these words in the bottom left-hand corner of the front page of each form.
